Information Release Authorization

The information provided in this application will be used in reference to your purchase/rental application. By submitting this application, you hereby authorize Your Neighborhood Management Group, Inc to use all information concerning your employment, residence, and banking to evaluate the application.

Applicant Signatures *

By entering your name(s) above, you hereby authorize Your Neighborhood Management Group (YNM) to investigate all statements contained in this application as may be necessary. You understand that you are waiving any privileges that you may have regarding requested information to release it to YNM for use in processing your application. An electronic copy of this form may be used in lieu of the original. The cost of the application process and the interview is $150 (not refundable) payable in the form of cash or money order to YNM. If for any reason the PSVS Board of Directors does not approve the application, the fee payable to the management company will not be refunded. Please note that a new application must be completed by all potential applicants each time a unit changes residents. The Approval Certificate will be delivered to the Applicant and must be retained for the entirety of the time living on the property.
